Description

Pride health is actively hiring a Physical Therapist to join our client's home health program in a growing care team serving patients.

Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Yes ($0.48/mile)

Weekly Hour Guarantee: 32

Must have qualifications/experience: Home health experience, Oasis and HCHB experience

What skills/attributes are nice to have, and will set a candidate apart? OASIS certification

Site1: KY-Shelbyville-40065; Coverage Areas: Shelby County, eastern Jefferson County, Spencer County.

Site2: KY-Mayfield-42066; Coverage Areas: Graves County and Calloway County.

Site3: KY-Hardinsburg-40143; Coverage Areas: Elizabethtown-Hardin County Hardinsburg-Meade County.

Site4: KY-Russellville-42276; Coverage Areas: Logan and Todd.
