Schedule: 3p-11:30p on a 5x8 schedule. 

Position Summary

The Licensed Practical Nurse (LPN) provides direct nursing care to veteran residents and their spouses in a long-term care environment. Working under the supervision of a Registered Nurse (RN), the LPN assists in planning, implementing, and evaluating resident care while ensuring compliance with professional nursing standards, facility policies, and HIPAA regulations. The LPN plays a critical role in promoting residents' health, safety, comfort, and quality of life.

Essential Duties and Responsibilities

• Administer prescribed medications and treatments according to physician orders.
• Monitor, assess, and document residents' health status and report significant changes to the RN or physician.
• Measure and record vital signs, weight, intake/output, and other clinical data.
• Assist with developing and implementing individualized care plans.
• Provide wound care, catheter care, and other nursing procedures within the LPN scope of practice.
• Observe residents for adverse reactions to medications or treatments and take appropriate action.
• Collect specimens for laboratory testing and perform routine nursing procedures.
• Assist residents with activities of daily living (ADLs), including bathing, dressing, mobility, and personal hygiene as needed.
• Maintain accurate and timely documentation in residents' medical records.
• Supervise and provide guidance to nursing assistants and support staff.
• Collaborate with interdisciplinary healthcare team members to ensure high-quality resident care.
• Promote resident dignity, independence, and emotional well-being.
• Adhere to infection control, safety, HIPAA, and regulatory requirements. 

Qualifications

• Current Pennsylvania Licensed Practical Nurse (LPN) license or temporary practice permit.
• Graduate of an approved practical nursing program.
• Knowledge of long-term care nursing practices and resident-centered care.
• Ability to communicate effectively with residents, families, physicians, and healthcare team members.
• Strong clinical assessment, documentation, and organizational skills.
• Ability to work rotating weekends and holidays as required.
