Travel RN Case Manager | Santa Rosa, CA
Position Overview
We are seeking an experienced RN Case Manager for a travel assignment in a fast-paced acute care hospital setting. The ideal candidate will have strong experience in care coordination, discharge planning, utilization management principles, and interdisciplinary collaboration. This position offers an excellent opportunity to work with a diverse adult and geriatric patient population while supporting positive patient outcomes through effective case management services.
Assignment Details
- Position: RN Case Manager
- Start Date: 07/13/2026
- Shift: Days, 5x8-Hour Shifts
- Shift Hours: 8:00 AM – 4:30 PM
- Contract Type: Travel Assignment
- Weekend Requirement: 2 weekends every 6 weeks
- On-Call Requirement: None
- Floating: Yes, based on department needs
Required Qualifications
Experience
- Minimum 2 years of Acute Care Hospital Case Management experience
- Recent experience in:
- Care Coordination
- Discharge Planning
- Fast-paced Acute Care environments
- Current work history within the last 2 years (Hard Stop)
Required Skills
- Epic EMR experience (Required)
- Experience managing a patient caseload of approximately 20 patients per day
- Strong communication and interdisciplinary collaboration skills
Certifications
- BLS Certification
Travel Experience
- Preferred, but first-time travelers may be considered based on clinical review and experience.
