Description
Position: Physical Therapist - Home Health
Location: Tampa, FL 33614
Shift: Day 5x8-Hour (08:00 - 17:00)
Contract Duration: 20 Weeks
Start date: Tentative
Travel Pay Rate:
$2,800 weekly
($30.10/hr w2 + $1,596/weekly stipend)
Job Summary:
We are seeking a compassionate and experienced Home Health Physical Therapist (PT) to provide skilled therapy services to patients in their homes throughout the Tampa, Florida area. The ideal candidate will be responsible for evaluating patients, developing individualized treatment plans, improving functional mobility, and promoting independence while delivering high-quality, patient-centered care. Candidates must have prior home health experience, with OASIS experience/training preferred, and be comfortable managing a diverse patient population in a home care setting.
Duties
- Perform comprehensive physical therapy evaluations and ongoing patient assessments in the home setting.
- Develop, implement, and modify individualized treatment plans based on patient needs and physician orders.
- Provide therapeutic interventions to improve strength, balance, mobility, endurance, and functional independence.
- Assess and treat patients with orthopedic, neurological, cardiopulmonary, post-surgical, and medically complex conditions.
- Educate patients and caregivers on home exercise programs, disease management, safety precautions, fall prevention, and equipment use.
- Evaluate and recommend assistive devices and durable medical equipment to optimize patient function and safety.
- Monitor patient progress and communicate changes in condition to physicians and interdisciplinary team members.
- Complete accurate and timely clinical documentation, including OASIS assessments when applicable.
- Ensure compliance with Medicare, Medicare Advantage, commercial insurance requirements, and agency policies.
- Recognize and report signs of physical, sexual, emotional, or psychological abuse in accordance with regulatory guidelines.
- Participate in care coordination and interdisciplinary team meetings to ensure continuity of care.
- Accurately complete billing and productivity requirements.
Requirements: - Active Florida Physical Therapist (PT) License.
- Current BLS Certification.
- 1 Year of Home Health Physical Therapy experience required.
- OASIS experience/training preferred.
- Strong assessment, documentation, and patient education skills.
- Valid driver's license and reliable transportation.
