Setting: SNF and Post Acute
Responsibilities: Evaluate patients functional abilities and develop individualized treatment plans. Provide hands-on therapy in accordance with physician orders and facility protocols. Document patient progress and maintain accurate treatment records. Collaborate with physical therapists, speech-language pathologists, nurses, and other healthcare professionals. Educate patients, families, and caregivers on techniques to support continued care and safety. Ensure compliance with all state and federal regulations
