Job Title: Travel LPN
Location: Rutland, VT
Shift: 8/hr Evening shift
Job Type: Contract (09 Weeks with the possibility of extension)

Position Summary:
The Licensed Practical Nurse (LPN) – Long-Term Care (LTC) provides direct patient care to residents in a long-term care setting. Responsibilities include administering medications, monitoring patient conditions, and supporting individualized care plans to ensure quality care and patient safety. The LPN works closely with RNs, physicians, and healthcare teams to deliver compassionate, patient-centered care.
 

Key Responsibilities:

  • Provide direct nursing care to residents in a long-term care facility
  • Administer medications and treatments as prescribed
  • Monitor patient conditions and report changes to RN/physician
  • Maintain accurate and timely documentation in EMR systems
  • Assist with activities of daily living (ADLs) and patient support
  • Perform wound care and support infection control practices
  • Collaborate with interdisciplinary teams to ensure continuity of care
  • Ensure compliance with facility policies and healthcare regulations

Requirements:

  • Active, unrestricted LPN License
  • BLS Certification (AHA preferred)
  • Minimum 1 year of LTC experience preferred
