Licensed Clinical Social Worker (LCSW / LMHC / LMFT) – Contract Assignment
Location: Blountstown, FL
Start Date: August 3, 2026
Contract Length: 13 Weeks
Schedule: Monday–Friday, 5x8-hour shifts (40 hours/week)
Position Overview
The Licensed Clinical Social Worker collaborates with the multidisciplinary care team to develop and implement patient treatment plans. This role provides case management, therapy, counseling, crisis intervention, and treatment planning for patients experiencing social, emotional, or behavioral health challenges across various age groups.
Key Responsibilities
- Conduct psychosocial assessments and evaluations
- Develop and implement individualized treatment plans
- Provide therapeutic counseling and crisis intervention
- Coordinate case management services and community resources
- Collaborate with physicians, nurses, and interdisciplinary team members
- Maintain accurate documentation and patient records
- Support discharge planning and continuity of care
Minimum Qualifications
- Active Florida license as one of the following:
- Licensed Clinical Social Worker (LCSW)
- Licensed Mental Health Counselor (LMHC)
- Licensed Marriage & Family Therapist (LMFT)
- Minimum 1 year of relevant experience
- Current BLS certification
