Description

Position Overview

We’re seeking a licensed Physical Therapist to provide skilled home health therapy services to patients across a local service area. In this role, you’ll evaluate patient needs, develop and update plans of care, and deliver treatment focused on restoring and maintaining functional mobility and overall wellbeing while collaborating with an interdisciplinary care team.

Key Responsibilities

  • Perform comprehensive patient assessments to determine therapy needs and appropriateness of care
  • Develop, document, and update Plans of Care (POC), progress notes, and related clinical documentation
  • Provide treatment for neuromuscular, neurological, and orthopedic conditions based on referral and evaluation
  • Educate patients/families on therapeutic procedures and safe mobility strategies
  • Train patients/families in transfers and safe use of adaptive equipment (walkers, wheelchairs, canes, braces, prosthetics, etc.)
  • Recommend and coordinate appropriate durable medical equipment as needed for home safety and function
  • Apply infection control and universal precautions during all patient care activities
  • Coordinate care and collaborate with physicians and the interdisciplinary team
  • Perform other duties as assigned within scope of practice

Required Qualifications

  • Active Physical Therapist license in the state of practice
  • Current CPR certification
  • Valid driver’s license, reliable transportation, and active insurance
  • 1–2 years PT experience in acute care or rehab setting (minimum)
  • Degree in Physical Therapy from an accredited program
  • Home Health experience
  • OASIS experience
  • Homecare Homebase (HCHB) experience

Preferred Qualifications

  • Prior home care experience (if not already reflected above)

Knowledge, Skills & Physical Requirements

  • Strong communication and interpersonal skills
  • Familiarity with durable medical equipment (DME)
  • Ability to manage a flexible schedule and travel locally for visits
  • Ability to perform physical job demands including lifting, bending, pushing/pulling, prolonged standing/walking
  • Ability to work under time-sensitive and occasionally stressful conditions
