Travel Home Care RN
Location: Morristown, NJ
Schedule: Monday–Friday | 8:00 AM – 4:30 PM

Job Overview:
We are seeking an experienced Home Care Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The ideal candidate will be experienced in home health assessments, care planning, wound care, medication administration, and patient education while working independently in the community.

Responsibilities:

• Perform comprehensive patient assessments in the home setting.
• Develop, implement, and update individualized Plans of Care (POC).
• Administer medications, IV therapy, wound care, catheter care, and other skilled nursing services.
• Complete accurate OASIS assessments and documentation using Homecare Homebase (HCHB).
• Educate patients and families on disease management, medications, nutrition, and home safety.
• Coordinate care with physicians, therapists, case managers, and other healthcare professionals.
• Maintain timely and accurate clinical documentation.
• Participate in interdisciplinary care planning and team meetings.

Requirements:

• Active RN license (New Jersey).
• Minimum 2 years of Home Health RN experience.
• OASIS experience – Required.
• Homecare Homebase (HCHB) experience – Required.
• Current BLS/CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Strong assessment, communication, and documentation skills.
• EPIC experience is strongly preferred.
