Description
7a-7p every Wednesday & Thursday and every other Friday, Saturday, Sunday.
ACTIVE License - LMSW or LCSW AND ACTIVE MS State License
Minimum 2 years of Acute Inpatient Medical Case Management experience that includes at least 1 year (in the last 3) in a LARGE Hospital (500+ beds, LEVEL 1/ACADEMIC Medical Center) (this does NOT include telephonic, utilization management, or therapy experience), - Previous travel in a level 1 facility (in the last 3 years) - Strong computer skills, Must be proficient in EPIC charting - Acute care/ED discharge planning experience, Recent (within the last two years) acute inpatient medical case management experience.
