Job Overview

We are seeking an experienced Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) and Rehabilitation Nursing Center in New York. This position is ideal for candidates with strong clinical skills, a commitment to quality patient care, and experience working in long-term care and rehabilitation settings.
 

Position Details

• Job Title: Licensed Practical Nurse (LPN)
• Location: New York
• Shift: Day Shift
• Schedule: 5 x 7.5-hour shifts
• Hours: 7:00 AM – 3:00 PM
• Weekend Requirement: Every other weekend
• Block Scheduling: Not Available

Minimum Qualifications

• Active New York LPN License
• Minimum 1.5 years of LPN experience
• Recent experience in Long-Term Care (LTC) and Rehabilitation Nursing
• Strong clinical assessment and patient care skills
• Ability to work every other weekend

Preferred Qualifications

• Experience using PointClickCare EMR
• Excellent communication and teamwork skills
• Ability to work efficiently in a fast-paced healthcare environment

Key Responsibilities

• Provide direct nursing care to residents in accordance with the care plan.
• Administer medications and treatments as prescribed.
• Monitor and document patient conditions and report changes to the healthcare team.
• Maintain accurate and timely documentation in the EMR system.
• Collaborate with physicians, nurses, therapists, and other interdisciplinary team members.
• Ensure compliance with facility policies, infection control standards, and patient safety protocols.
• Deliver compassionate, high-quality care while promoting resident comfort and well-being.
