ACLS, BLS (AHA), BronxCare Hospital Center - Grand Concourse Campus De-escalation Training, PALS

Experience Required

  • Minimum 1 year of psychiatric nursing experience.
  • Medical-surgical or higher-acuity medical experience is preferred.
  • ED experience is not required.
  • Strong de-escalation and restraint experience is preferred.

 

Position Requirements

  • No first-time travelers.
  • This must be your primary job.
    • No part-time or second jobs.
    • Per diem positions are considered only on a case-by-case basis, and the unit will not schedule around them.
  • RTO (Requested Time Off) must be submitted before the contract starts. Requests after the start date will not be considered.

Unit Overview

  • 40-bed psychiatric emergency unit
    • Adult and adolescent patients (travelers work both populations)
  • Patient volume: Approximately 15–45 patients per day
  • Dedicated triage/fast-track area
  • 24/7 security on-site

Staffing & Ratios

  • RN-to-patient ratio: 1:6–12, depending on acuity.
  • Typical staffing:
    • 2 Triage RNs
    • 4 Adult RNs
    • 1 Pediatric/Adolescent RN
    • 1 Charge RN
  • Travelers generally do not serve as Charge Nurse.

Required Certifications

Must have:

  • BLS
  • ACLS
  • PALS

Plus one of the following before starting:

  • Handle With Care
  • TRUST Training
  • CPI (Crisis Prevention Institute)
  • PMCS
  • MOAB

 

Patient Population

Common diagnoses include:

  • Schizophrenia
  • Bipolar disorder
  • Major depressive disorder
  • Anxiety
  • Trauma-related disorders
  • Behavioral crises

Patients are generally medically stable; unstable patients are transferred to the main hospital.

Nursing Responsibilities

  • PO and IM medication administration
  • Common medications:
    • Haldol
    • Zyprexa
    • Ativan
  • No routine IV therapy.
  • Recognize psychiatric crises early.
  • Use verbal de-escalation and behavioral interventions.
  • Remain calm and compassionate during behavioral emergencies.

Support Staff

  • PCTs perform:
    • Phlebotomy
    • EKGs
    • 1:1 observation
    • Transport
    • ADLs
    • Contraband searches
    • Property inventory
  • Respiratory Therapy available 24/7.
  • Portable telemetry used only in limited situations; RNs should recognize life-threatening rhythms.

Scheduling

  • Scheduled based on unit needs.
  • No block scheduling.
  • Every other weekend required.
  • Holiday rotation possible.
  • No call requirement.

Floating

Travelers float first and may rotate to:

  • Adult psych
  • Adolescent psych
  • Detox
  • LRC
  • Inpatient units

Orientation includes shifts in each area.

Dress Code

  • Royal Blue scrubs
