Qualifications
Description
We’re seeking a Physical Therapist (PT) to provide in-home patient care across a designated territory. You’ll evaluate, treat, and educate patients to help restore function, improve mobility, and support safe independence at home while collaborating with an interdisciplinary care team.
Key Responsibilities
- Perform patient evaluations and determine appropriateness/needs for therapy services
- Develop and maintain plans of care (POC) and adjust based on ongoing assessment
- Provide treatment for neuromuscular, neurological, and orthopedic conditions
- Document all required clinical notes including progress notes and related orders/updates
- Educate patients and caregivers on therapeutic procedures, safety, and home exercise programs
- Train patients/families on transfers and safe use of assistive devices (walker, wheelchair, cane, braces, prosthetics, etc.)
- Apply infection control and universal precautions in all patient care activities
- Support/oversee other team members when applicable
Requirements
- Active Physical Therapist license (state-specific)
- Current CPR certification
- Valid driver’s license + reliable transportation + insurance
- 1–2 years PT experience (acute care or rehabilitation setting)
- Bachelor of Science in Physical Therapy from an accredited program (per client requirement)
- Homecare Homebase (HCHB) experience
- OASIS experience
- Home Health experience
Preferred
- Prior home care/home health background
