LPN – Long-Term Care (LTC)

Shift: Full-Time Day/Night Rotation (5x8)
$1,520.00/week
Duration: 13

Requirements:

• Active New York LPN License.
• BLS certification through AHA or RQI.
• Minimum 1 year of LPN experience in a Long-Term Care (LTC) facility.
• Every other weekend (EOW) and holiday availability required.
• Must attend full-shift orientation starting on a Tuesday or Thursday.

Responsibilities:

• Provide direct and indirect nursing care to adult and geriatric residents under RN supervision.
• Perform ongoing physical and psychosocial assessments and report changes to the RN.
• Assist in developing, implementing, and updating individualized care plans.
• Administer medications, treatments, and monitor resident responses.
• Collaborate with physicians and the interdisciplinary team to ensure quality resident care.
• Maintain accurate documentation and adhere to facility policies, safety standards, and infection control practices.
• Participate in staff orientation, continuing education, and quality improvement initiatives.
