Description
Position Overview
Healthcare Support is seeking a Physical Therapist (PT) for a home health contract assignment. This role focuses on delivering skilled PT services in the home setting and supporting patient progress through evidence-based treatment plans and thorough documentation.
Schedule & Assignment Details
- Hours: 40 hours/week
- Shift: Days (8-hour shifts)
- Weekend rotation: Required
- On-call: Not required
- Contract length: 13 weeks
- Start date: Week of August 12, 2026 (per job start date on file)
Coverage Area
- Home health territory in North Napa County (exact facility details not included)
Key Responsibilities
- Complete in-home PT evaluations and develop individualized plans of care
- Provide skilled therapeutic interventions to improve mobility, strength, balance, and functional independence
- Educate patients/caregivers on home exercise programs, safety, fall prevention, and equipment use
- Coordinate with the interdisciplinary team to support continuity of care
- Maintain timely, accurate documentation per home health standards and payer requirements
Performance Expectations
- Productivity: Minimum 30 points/week
- Documentation deadline: Completed by 10:00 AM the following day
Required Qualifications
- Active Physical Therapist license (state requirements apply)
- Minimum 1 year of recent Home Health PT experience
- Strong clinical judgement, time management, and patient communication skills
- Ability to work independently in the field and manage a daily route/visit schedule
