Acute Care Social Worker – Fort Pierce, FL
$2,086 Weekly Gross | 5x8 Day Shifts | Acute Care Hospital
TravSource is seeking an experienced Acute Care Social Worker for an exciting travel opportunity at a 344-bed acute care hospital in Fort Pierce, Florida. If you have a strong background in adult acute care hospital case management, this is an excellent opportunity to join a collaborative care management team.
Position Details
Location: Fort Pierce, FL 34954
Weekly Gross Pay: $2,086
Schedule: 5x8 Day Shifts
Hours: 8:00 AM – 4:30 PM
Weekend Rotation: Every 3rd Weekend
Setting: 344-Bed Acute Care Hospital
Caseload
Manage approximately 8–9 complex case management patients in collaboration with an RN Case Manager.
Coordinate discharge planning for approximately 25 patients.
Collaborate with physicians, nursing, therapy, and community partners to ensure safe and timely patient transitions.

Care Team Assignment (CTA)
Required Experience
Acute Care Adult Hospital Case Management experience is required.
Strong discharge planning and care coordination experience.
Experience working with interdisciplinary healthcare teams.
Knowledge of post-acute placement, insurance authorizations, and community resources.
Excellent communication and documentation skills.
