• Position: Physical Therapist
• Location: Rock Hill, South Carolina
• Facility: Atrium Health At Home York
• Shift: Day Shift (8:00 AM – 5:00 PM)
• Hours: 40 Hours/Week
• Contract Length: 89 Days (Approximately 13 Weeks)

Facility Information

• Home Health Services
• Adult patients only
• Approximately 6 points/visits per day
• Covers York and Lancaster Counties
• May occasionally float to Union or Mecklenburg County

Candidate Requirements

• Minimum 1 year of Home Health Physical Therapy experience
• First-time travelers accepted if experienced in Home Health
• Must live more than 50 miles from the home unit

Required Licensure & Certifications

• Active Physical Therapist State License
• BLS Certification
• Valid Driver's License
• Proof of Auto Insurance
• Minimum Bachelor's Degree

Required Skills

• Homecare Homebase (HCHB) experience (Required – No Exceptions)
• OASIS-E assessment experience
• IV care
• PleurX catheter care
• Wound VAC
• Wound care
• Patient assessment
• Disease education
• Chronic disease management
• Geriatric/Adult patient care
• Maintenance notes/documentation

Technology

• EMR: Homecare Homebase (Required)
• Samsung Note 9 phones

Orientation

• New Employee Orientation (NEO)
• 5 days of orientation

Additional Information

• Grey scrubs (Therapy)
• Guaranteed 40 hours per week
