Description

An RN Case Manager is a registered nurse responsible for coordinating and managing patient care across the healthcare continuum. They assess patient needs, develop individualized care plans, coordinate with multidisciplinary teams, facilitate discharge planning, and ensure patients receive appropriate, cost-effective, and high-quality care. RN Case Managers work in hospitals, home health, hospice, insurance companies, rehabilitation facilities, and community health settings.
