RN – Hospice | Roles & Responsibilities

• Provide compassionate nursing care to hospice patients in home, facility, or inpatient settings.
• Perform comprehensive patient assessments and develop individualized hospice care plans.
• Complete hospice admissions, including physical assessments, medication reconciliation, and required documentation.
• Coordinate care with physicians, case managers, social workers, chaplains, and interdisciplinary hospice team members.
• Manage pain and symptoms to promote patient comfort and quality of life.
• Administer medications and treatments according to physician orders and hospice protocols.
• Educate patients and families on disease progression, symptom management, end-of-life care, and available resources.
• Respond to after-hours and on-call patient needs, providing timely clinical interventions and support.
• Maintain accurate documentation in accordance with hospice regulations and agency policies.
• Ensure compassionate, patient-centered care while supporting families throughout the end-of-life process.

Required Documents / Certifications

• Active RN License
• BLS Certification
• Hospice Nursing experience
• Hospice Admissions experience (required)
• Ability to participate in a 7 On / 7 Off On-Call Schedule
• Strong assessment, communication, and end-of-life care skills
