*Work Setting Skills REQUIRED: Acute Hospital & Outpatient Clinic
*Pre-Transplant/Candidates' Skills REQUIRED: Pre-transplant evaluation and assessment
*Clinical assessment and Skills REQUIRED: Interpretation of lab results and diagnostic tests related to transplant, Medical history of potential/actual donors, Monitors vital signs, weight, fluid balance, and graft function & Reviews and interprets relevant diagnostic and laboratory data
*Professional Knowledge and Skills REQUIRED: Coordinate all patient care needs before, during and after transplantation, Patient Confidentiality/HIPAA, Patient/Family Education, Schedules and tracks pre- and post-transplant testing, Transplant center protocols, Transplant team collaboration, Understanding of post-transplant health maintenance and long-term care, Universal precautions & Waitlist criteria
Certs: BLS

-Pre-Transplant/Candidates Skills: Absolute contraindications to transplant, Assists with organ offers, acceptance, and recipient preparation, Organ allocation process & Relative contraindications to transplant
-Post-Transplant/Recipients Skills: Coordinates post-transplant care with team, Monitors post-transplant patient progress, Offers psychosocial support and education on lifestyle adjustments to patient and family & Teaches medication adherence and post-op care plans
-Clinical assessment and skills: Familiarity with organ-specific diseases (e.g., ESRD, cirrhosis, heart failure), Identifies signs/symptoms of transplant complications (infection, rejection, etc.), Performs comprehensive physical and psychosocial assessments & Recognition and management of transplant complications (infection, rejection, etc.)
-Medication Administration Skills: Coordinates medication adjustments post-transplant with provider, Reviews pre-transplant medication regimens for contraindications, Teaches medication adherence and self-administration & Understanding of pharmacology relevant to transplant patients
-Regulatory & Administrative Compliance Skills: Tracks patient outcomes and reports per regulatory standards
-Professional Knowledge and Skills: Assisting with scheduling of the OR
Certs: Chemo (ONS/ONCC)
JOB DETAILS:
RATIOS: Daily Census could be 8, but visits could be maybe 20 if you are counting all the interactions with all parties on behalf of and including the patient.
SHIFT: *40 Hours Can Be Offered
Will Accept 12 Weeks
8-HOUR DAYS; Monday - Friday 08:00 - 16:30

PATIENT DIAGNOSES: hematologic malignancies
SPECIAL PROCEDURES / UNIT DETAILS: Peripheral blood transplant cellular therapy. Transplant Coordinators have patient cases that they follow from candidate identification through the workup/evaluation, cell harvest, and cellular therapy infusion or cell transplant. The patients are in different phases of this process, and a coordinator could have an assignment of 8+ patients at one time. Coordinators meet with patients several times and schedule all their appointments according to a required timeline. But there are not a daily census or visits per day. Coordinators attend meetings with doctors and other staff throughout the week. Coordinators must be organized and able to work autonomously.

I have been attempting to reach them for over a month with absolutely no response! They lose paperwork (tests, immunizations, background checks and fingerprints) resulting in even more lost work.
