Description
Responsibilities:
Perform comprehensive patient assessments and develop individualized care plans.
Complete OASIS assessments and maintain accurate, timely documentation.
perform other job duties as assigned.
Requirements:
Active RN license
ASN or BSN required
BLS certification required
OASIS experience is required
Minimum 2 years of Home Health RN experience
