Pride Health is hiring an RN Home Health role in Indianapolis, IN, to support our client in a Healthcare Setting.

Title - RN Home Health

Location - Indianapolis, IN

Schedule: Monday–Friday | 8:00 AM–4:30 PM
Weekly Guarantee: 40 Hours
Coverage Area: Marion, Hamilton, and Johnson Counties
On-Call/Weekend Requirement: None

Productivity Points - 25 -30 points/ week

Key Responsibilities

• Complete OASIS Start of Care (SOC) assessments and documentation accurately and timely.
• Perform patient recertifications and discharge visits in accordance with agency guidelines.
• Develop, implement, and evaluate individualized care plans.
• Provide medication setup, education, and management for patients and caregivers.
• Deliver skilled wound care treatments and monitor healing progress.
• Collaborate with physicians, therapists, and interdisciplinary team members to coordinate patient care.
• Maintain comprehensive documentation within the Homecare Homebase (HCHB) system.
• Educate patients and families on disease management, medications, safety, and home care needs.
• Ensure compliance with all regulatory and agency standards.

Required Qualifications

• Active Registered Nurse (RN) license.
• Previous home health nursing experience.
• OASIS assessment experience, including Start of Care, Recertification, and Discharge documentation.
• Proficiency with Homecare Homebase (HCHB).
• Strong clinical assessment and critical thinking skills.
