Description

Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

 

Compensation:

  • Local Pay: $50–$59/hour (W-2)
  • Travel Pay: $2,500–$2,600 gross per week
  • Mileage Reimbursement: $0.48/mile (from first patient to last patient)

Schedule:

  • Monday–Friday
  • 40 hours per week
  • Weekly Hour Guarantee: 32 hours (25 productivity points)
  • Day Shift
  • On-Call Requirement: Minimal – 1 day per week and 1 weekend per month
     

Patient Care Setting: Home Health

 

Job Responsibilities:

  • Perform comprehensive patient assessments and develop individualized care plans.
  • Complete accurate and timely OASIS documentation.
  • Provide skilled nursing care, including medication administration, wound care, disease management, and patient education.
  • Monitor patient progress and collaborate with physicians and the interdisciplinary care team.
  • Educate patients and caregivers on medications, treatment plans, and home safety.
  • Maintain timely and accurate clinical documentation while meeting productivity expectations.

Required Qualifications

  • Active Registered Nurse (RN) license
  • Current BLS/CPR Certification
  • Minimum 3 years of Home Health RN experience
  • Minimum 3 years of OASIS documentation experience
  • Valid driver's license and reliable transportation

Preferred Skills:

  • Excellent clinical assessment and critical thinking skills
  • Strong communication and patient education abilities
  • Ability to work independently and efficiently manage a daily schedule
  • Experience coordinating care with multidisciplinary healthcare teams

Dress Code: Royal Blue or Black scrubs

 

Locations & Coverage Areas (Some locations have 2 open positions due to the large patient population, and some has 1):

  • Richmond, KY 40475 – Agency Service Area
  • Frankfort, KY 40601 – Franklin, Mercer, Woodford, and Anderson Counties
  • Owensboro, KY 42303 – Owensboro (primarily), Ohio County, and Madisonville as needed
  • Franklin, KY 42134 – Simpson County
  • La Grange, KY 40031 – Jefferson, Oldham, Henry, Trimble, and Carroll Counties
  • Shelbyville, KY 40065 – Shelby County and parts of Eastern Jefferson County
  • Louisville, KY 40272 – South & Metro Louisville (including ZIP codes 40214, 40215, 40216, 40258, 40272, 40118, and surrounding areas)
