Job Title: Registered Nurse Case Manager
Location: Mission Viejo, CA
Shift : 8 hour /Day
Job Type: Contract (13 weeks with possible extension)

Position Summary:We are actively seeking an experienced Travel Registered Nurse (RN) – Case Manager to join a highly respected acute care hospital in Mission Viejo, CA. This 13-week travel assignment is ideal for an RN with recent acute care case management experience who excels in care coordination, discharge planning, and multidisciplinary collaboration. Key Responsibilities:
- Coordinate patient care throughout hospitalization.
- Perform comprehensive patient assessments.
- Develop and implement individualized discharge plans.
- Collaborate with physicians, nurses, social workers, and interdisciplinary teams.
- Arrange DME, SNF placement, home health services, and transportation.
- Participate in family meetings and treatment planning.
- Ensure compliance with CMS, HIPAA, Joint Commission, and hospital regulations.
Requirements:
- Active California RN license
- BLS by AHA
- COVID card (Medical/Religious exemptions are accepted)
- Minimum 2 years of recent Acute Care RN Case Management experience
- Recent Care Coordination experience within the last 3 months
- Previous Travel Nursing experience required
- Epic EMR experience required
- Experience with discharge planning and care coordination in an acute hospital setting
