Physical Therapist – Home Health Travel - $2,400.00/wk

Shift

• 8-hour Days
• Monday–Friday, 8:00 AM – 5:00 PM
• No weekends or holiday rotation
• 40 guaranteed hours per week
• 13-week contract

Requirements

• Active North Dakota Physical Therapy License
• BLS certification
• Current Driver's License and Auto Insurance
• Home Health experience preferred
• OASIS documentation experience preferred
• First-time travelers considered
• Must live more than 50 miles from the facility
• Cannot have worked full-time, part-time, or PRN for any CommonSpirit, CHI, or Dignity Health facility within the past 12 months

Responsibilities

• Provide skilled physical therapy services in patients' homes
• Complete Start of Care (SOC), Recertifications, and Resumption of Care (ROC) visits
• Perform comprehensive patient evaluations and develop treatment plans
• Deliver approximately 5–6 routine visits per day (or 1 admission plus 3 additional visits)
• Complete OASIS documentation accurately and timely
• Educate patients and caregivers on mobility, safety, and home exercise programs
• Perform occasional Hospice safety evaluations or one-time teaching visits when needed
• Coordinate care with physicians and the interdisciplinary healthcare team
