Ambulatory Care / Care Manager RN – Juneau, AK

Location: Juneau, AK
Start Date: 08/17/2026
Contract Length: 13 Weeks

Shift Options:

Monday–Friday | 8:00 AM – 5:00 PM
Monday–Friday | 10:00 AM – 7:00 PM
Tuesday–Saturday | 8:00 AM – 5:00 PM

We are seeking an experienced Ambulatory Care / Care Manager Registered Nurse for a 13-week travel assignment in Juneau, Alaska.

Job Responsibilities:

Coordinate and manage patient care across the continuum of care
Develop, implement, and monitor individualized care plans
Collaborate with providers, patients, families, and interdisciplinary teams to ensure quality outcomes
Provide patient education, discharge planning, and care coordination
Maintain accurate documentation and ensure compliance with organizational standards
Advocate for patients and assist with accessing appropriate healthcare resources

Requirements:

Active Alaska RN License required at the time of submission
Associate's Degree in Nursing from an accredited program
Current, unrestricted Alaska Registered Nurse license
Current BLS Certification
Minimum 1 year of RN experience
Case Management experience within the last 2 years is required
Proof of current Flu Vaccination is required
