Job Title: Physical Therapist - Home Care
Location: Dunedin, FL 34698
Job Type: 20-week contractual + extensions
Shift:Day 5x8-Hour (08:00 - 17:00)

Patient Age Groups: Adults, Geriatrics

Compensation: $55–$60 per hour, based on experience and qualifications.

Position Summary:
We are seeking a dedicated and skilled Physical Therapist to join our healthcare team, committed to providing exceptional patient care across diverse settings. The ideal candidate will possess a comprehensive understanding of physiotherapy principles, anatomy, and physiology, enabling them to develop and implement effective care plans tailored to individual needs.

Key Responsibilities:

• Perform comprehensive physical therapy evaluations in patients' homes.
   
• Develop individualized treatment plans based on patient needs and physician orders.
   
• Assess mobility, strength, balance, gait, and functional abilities.
   
• Provide therapeutic exercises and rehabilitation programs.
   
• Train patients in the use of assistive devices such as walkers, canes, and wheelchairs.
   
• Educate patients and family members on exercises, safety precautions, and disease management.
   
• Monitor patient progress and modify treatment plans as needed.
   
• Assess home environments and recommend safety modifications to prevent falls and injuries.
   
• Maintain accurate and timely documentation of evaluations, treatments, and patient progress.
   
• Communicate regularly with physicians, nurses, occupational therapists, speech therapists, and other

Note:
Home skilled visits - 6-8 per day (MINIMUM of 6 points per day at managers discretion may increase by 1 point)
Every 3rd weekend is required.
Laptop, MiFi for internet access, Portable printers.

Required Qualifications:

• Active Florida (FL) Physical Therapist (PT) License
• BLS/CPR Certification
• Minimum 1 year of Home Health Physical Therapy experience
• Valid driver's license and reliable transportation (required)
