On-Call Registered Nurse (RN) - Home Health
Schedule: Full-time • On-call required • Local travel/field visits
Location: Denver, CO area (On-site/field-based)
On-Call Registered Nurse (RN) - Home Health Summary:
- Provide skilled nursing visits in the home; manage and update Plan of Care (POC)
- Educate patients/families, coordinate with interdisciplinary team
- Maintain compliant documentation and continuity of care
On-Call Registered Nurse (RN) - Home Health Requirements:
- Active RN license
- 2+ years home health experience (required)
- Homecare Homebase (HCHB) experience (required)
- OASIS experience (required)
- CPR + driver’s license + reliable transportation/insurance
