LPN – Long-Term Care / Skilled Nursing | Roles & Responsibilities

• Provide direct nursing care to residents in a long-term care and skilled nursing setting.
• Assess, monitor, and document resident conditions, reporting changes to the healthcare provider.
• Administer medications, treatments, and wound care in accordance with physician orders and facility policies.
• Care for residents with dementia, Parkinson's disease, major depressive disorder (MDD), and other chronic medical conditions.
• Coordinate and supervise Certified Nursing Assistants (CNAs) to ensure quality resident care.
• Assist with admissions, discharges, and care planning.
• Monitor resident safety, infection prevention, and fall prevention measures.
• Educate residents and families regarding medications, treatments, and ongoing care plans.
• Document all nursing care accurately using PointClickCare (PCC).
• Collaborate with physicians, therapists, and interdisciplinary team members to promote optimal resident outcomes.

Required Documents / Certifications

• Active LPN License
• BLS Certification
• Long-Term Care (LTC) or Skilled Nursing Facility (SNF) experience
• Experience caring for residents with dementia and other chronic conditions
• Experience using PointClickCare (PCC) (preferred)
• Ability to work 12-hour day shifts, including every other weekend and holiday rotation
