Description

Hospital Social Worker (LCSW) – Acute Care Hospital | Roles & Responsibilities

  • Provide psychosocial assessments for hospitalized patients and develop individualized care and discharge plans.
  • Coordinate discharge planning, transitions of care, and post-acute placement for patients with complex medical and psychosocial needs.
  • Collaborate with physicians, nurses, case managers, therapists, and community agencies to ensure continuity of care.
  • Assess patients for social, emotional, financial, and environmental barriers affecting recovery and discharge.
  • Provide crisis intervention, counseling, emotional support, and patient/family education.
  • Coordinate referrals for rehabilitation, skilled nursing facilities, home health, hospice, behavioral health, and community resources.
  • Participate in interdisciplinary rounds and advocate for patient-centered care.
  • Assist with advance directives, guardianship issues, and other psychosocial needs as appropriate.
  • Maintain accurate documentation in accordance with hospital policies, regulatory standards, and professional practice guidelines.
  • Ensure compliance with patient confidentiality, safety, and discharge planning requirements.

Required Documents / Certifications

  • Active Georgia Licensed Clinical Social Worker (LCSW) License
  • BLS Certification (AHA)
  • Minimum 2 years of recent Acute Care Hospital Social Work experience
  • Hospital-based discharge planning experience
  • Experience with psychosocial assessments and care coordination
  • Strong interdisciplinary communication and crisis intervention skills
