Schedule: Monday–Friday | 8:30 AM – 5:00 PM | Weekend & Evening On-Call Rotation Required

Contract Duration: 13 Weeks with possible extension

Position Overview

Seeking an experienced Registered Nurse (RN) for a Home Health assignment in Meridian, Idaho. This opportunity is ideal for an RN with strong home health experience who is confident in OASIS documentation, Medicare homebound criteria, and skilled nursing assessments. The position offers a Monday–Friday day schedule with rotating weekend and evening on-call responsibilities.

Key Responsibilities

Provide comprehensive nursing care to patients in their homes.
Complete patient assessments and develop individualized care plans.
Perform skilled nursing procedures and treatments.
Accurately complete OASIS documentation.
Determine Medicare homebound eligibility and appropriate skilled services.
Educate patients and caregivers on disease management and treatment plans.
Coordinate care with physicians, therapists, and interdisciplinary healthcare teams.
Document all patient care using the EPIC electronic medical record system.
Maintain compliance with agency policies and regulatory standards.
Participate in weekend and evening on-call rotation as scheduled.

Skills:

Excellent communication, assessment, and critical thinking skills.
Ability to work independently while managing multiple patient visits.
Strong knowledge of Medicare homebound criteria.
Experience with OASIS assessments and documentation.
Proficiency in EPIC or similar electronic medical record systems.

Assignment Highlights

Day Shift: Monday–Friday, 8:30 AM – 5:00 PM
Approximately 5–6 patient visits per day
Travel typically maintained within 15–20 minutes between visits
Opportunity for overtime
Ability to begin and end the workday from home after orientation

Qualifications

Active Registered Nurse (RN) license.
Minimum 5 years of Registered Nurse experience required.
Recent 2 Years Home Health experience is required.
AHA - BLS.
