Job Description – Licensed Clinical Social Worker (LCSW)
Responsibilities:
- Conduct biopsychosocial assessments.
- Provide case management, utilization review, and care coordination.
- Participate in care conferences and collaborate with multidisciplinary teams to ensure continuity of care.
- Refer patients to appropriate disciplines and community resources.
- Assess and intervene in high-risk situations, including suicide, homicide, self-harm, and patient safety concerns.
- Collaborate with Child Protective Services when appropriate.
- Provide support for drug/alcohol use disorders and treatment options.
- Apply knowledge of cultural issues and adhere to the NASW Code of Ethics/Standards of Home Health Care.
Required Knowledge & Experience:
- Biopsychosocial assessment.
- Case management and utilization review.
- Care coordination and collaborative care.
- Child Protective Services.
- High-risk patient intervention and vulnerability/safety risk.
- Suicide, homicide, and self-harm assessment.
- Drug/alcohol use and treatment options.
- Multidisciplinary team collaboration.
- Experience in Acute Hospital, Outpatient Clinic, Psychiatric Hospital, and Pediatrics settings.
