Description

Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.


Compensation:

  • Local Pay: $50–$59/hour (W-2)
  • Travel Pay: $2,500–$2,600 gross per week
  • Mileage Reimbursement: $0.50/mile (from first patient to last patient)

Location: Farmington Hills, MI 48334

Mileage Reimbursement: $0.51/mile (from first patient to last patient)

 

Schedule:
✅ 40 Hours Guaranteed Weekly
✅ Monday–Friday | 8:00 AM – 4:30 PM (EST)
✅ On call: 1 weekday per week & 1 weekend per month

 

Coverage Area:
Serving patients throughout:
• Oakland County
• Wayne County
• Livingston County
• Washtenaw County
• Macomb County
• Genesee County

 

Key Responsibilities:
✔️ Comprehensive in-home patient assessments
✔️ Skilled nursing visits and medication management
✔️ Wound care & post-acute follow-up
✔️ Develop and manage individualized care plans
✔️ Patient & caregiver education
✔️ Care coordination with interdisciplinary teams
✔️ Accurate HomeCare HomeBase (PointCare) documentation
✔️ Independent clinical decision-making and patient advocacy

 

Qualifications:

  • Active RN License
  • Minimum 1 year of Home Health experience
  • HomeCare HomeBase (PointCare) EMR experience required
  • Current BLS Certification
  • Valid driver's license & reliable transportation
  • Ability to work independently in patients' homes

Ideal Candidate

  • Strong clinical assessment and judgment
  • Excellent communication and organizational skills
  • Experience with wound care, chronic disease management, and post-acute care

Dress Code: Navy Blue Scrubs
