Job Role: Social Services – MSW (Masters of Social Work) – Case Management
Facility Name: Providence Saint John's Health Center
Facility Address: 2121 Santa Monica Blvd, Santa Monica, CA 90404
Requirements
- Master's degree in Social Work (MSW) required; LCSW preferred.
- Minimum 2 years of acute medical hospital inpatient social work experience performing care coordination and discharge planning.
- Travel experience required (first-time travelers may be considered).
- Proficiency with Epic charting system.
- Experience with biopsychosocial assessments, crisis intervention, discharge planning, care coordination, family meetings, and treatment planning.
- Knowledge of HMOs, IPAs, advance directives, protective services, mental health crises, homelessness resources, adoption/foster care, and substance use treatment resources.
- Experience coordinating post-acute services, home care, DME, rehab, and long-term care referrals.
- Weekend rotation required (2 weekends every 4 weeks).

Staff constantly switched. My documents weren't sent to the school, resulting in a months lost pay. It is IMPOSSIBLE to get on touch with anyone to answer questions. I have been attempting to reach them for over a month with absolutely no response! They lose paperwork (tests, immunizations, background checks and fingerprints) resulting in even more lost work.
