This position is a 3rd shift schedule. 

10:45pm-7:15am 

 

Position Summary

The Registered Nurse (RN) provides comprehensive nursing care to residents in a skilled nursing and long-term care setting. The RN is responsible for assessing residents' health status, developing and implementing individualized care plans, administering medications and treatments, supervising nursing personnel, coordinating interdisciplinary care, and ensuring compliance with professional nursing standards and regulatory requirements. The RN promotes quality of life, safety, dignity, and compassionate resident-centered care.

Essential Responsibilities

  • Assess, plan, implement, and evaluate individualized nursing care for residents with acute, chronic, and rehabilitative healthcare needs.
  • Perform comprehensive nursing assessments and monitor residents for changes in physical, mental, and emotional condition.
  • Administer medications, intravenous therapies, blood products (as authorized), treatments, and other nursing interventions in accordance with physician orders and nursing standards.
  • Develop, update, and evaluate individualized care plans in collaboration with physicians, therapists, social workers, dietary staff, and other interdisciplinary team members.
  • Supervise, delegate, and evaluate the work of Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs).
  • Coordinate admissions, transfers, discharges, and continuity of care for residents.
  • Respond promptly to medical emergencies, initiate appropriate nursing interventions, and communicate with healthcare providers as needed.
  • Monitor infection prevention practices and implement appropriate isolation and safety precautions.
  • Educate residents, families, and staff regarding disease processes, medications, treatments, preventive care, and health promotion.
  • Maintain accurate, complete, and timely documentation in the electronic medical record in accordance with legal, regulatory, and organizational standards.
  • Participate in quality assurance, performance improvement initiatives, and regulatory compliance activities.
  • Serve as the charge nurse when assigned, coordinating resident care and providing clinical leadership during the assigned shift.
  • Ensure compliance with federal and state regulations, professional nursing standards, HIPAA requirements, and organizational policies and procedures.
  • Promote a safe, respectful, and therapeutic environment that supports residents' dignity, independence, and overall well-being.

Qualifications

  • Current licensure as a Registered Nurse in the Commonwealth of Pennsylvania.
  • Graduation from an accredited school of nursing.
  • Current CPR/BLS certification (or ability to obtain upon hire).
  • Knowledge of skilled nursing, long-term care, rehabilitation, and geriatric nursing principles.
  • Strong clinical assessment, critical thinking, communication, and leadership skills.
  • Ability to supervise nursing personnel and work effectively within an interdisciplinary healthcare team.
  • Proficiency in electronic health record documentation.

Work Environment

  • Skilled nursing and long-term care setting providing care to veterans and other eligible residents with complex medical and rehabilitative needs.
  • Rotating shifts, weekends, holidays, on-call responsibilities, and overtime may be required based on operational needs.
  • Frequent standing, walking, bending, lifting, repositioning residents, and direct patient care activities are essential functions of the position.
