Requirements

• Active RN license
• Minimum 1 year of Certified Home Health experience required
• Minimum 1 year of Home Health Case Management experience required
• Experience must be with a Certified Home Health Agency
• Hospice, private duty, or non-certified home health case management alone does NOT qualify
• Reliable transportation and willingness to travel between patient homes

Responsibilities

• Complete Start of Care (SOC), admissions, and ongoing home visits
• Manage patient caseload from admission through discharge
• Develop and update individualized care plans
• Provide patient and family education
• Coordinate care with physicians and interdisciplinary team
• Complete timely electronic documentation

Coverage Area

Patient visits throughout the Greater Boston area, including:

• Newton, Wellesley, Framingham, Natick, Brookline, Belmont, Watertown, Marlborough, Hopkinton, Hudson, Weston, Westwood, and surrounding communities.

• Weekend rotation only once every 6 weeks

Ideal Candidate: An experienced Certified Home Health RN Case Manager who is comfortable managing an independent caseload, completing admissions, and delivering high-quality home health care in the community.
