Job Description – Home Health Physical Therapist Assistant (PTA)

Location: West Portland, OR & Surrounding Areas
Setting: Home Health
Shift: 8-Hour Days (No 4x10s or 3x12s)
Start Date: ASAP

Requirements

Minimum 1 year of Skilled Home Health PTA experience required.
First-time travelers accepted.
BLS Certification required.
Pending Oregon license accepted.
Must have reliable personal transportation.
Primary COVID-19 vaccination series plus one booster required.

Job Duties

Provide physical therapy treatments under the supervision of a Physical Therapist in patients' homes.
Assist patients in improving mobility, strength, balance, and functional independence.
Implement individualized treatment plans and document patient progress using Epic Remote Client and Rover.
Educate patients and caregivers on home exercise programs and safety techniques.
Travel between patient homes, typically seeing up to 6 patients per day while meeting productivity standards.
Collaborate with the interdisciplinary healthcare team to ensure quality patient care.

Additional Information

No weekends or on-call required.
No floating between units.
Free parking and IRS standard mileage reimbursement provided.
Facility may call off up to one shift per pay period.
