Location: Trenton, NJ
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
Job Overview
We are seeking an experienced Registered Nurse (RN) Case Manager to join a leading healthcare organization in New Jersey. This field-based position focuses on coordinating care for members through comprehensive assessments, care planning, and collaboration with providers to improve health outcomes.
The Case Manager will conduct both telephonic and face-to-face assessments in members' homes, hospitals, behavioral health facilities, and skilled nursing facilities while ensuring timely, cost-effective, and member-centered care.
Responsibilities
- Conduct comprehensive clinical assessments of members' healthcare needs.
- Develop, implement, and monitor individualized care plans.
- Coordinate care with physicians, hospitals, community providers, and caregivers.
- Perform home visits and facility visits as needed.
- Facilitate safe hospital discharges and transitions of care.
- Identify and address social determinants of health, including housing, transportation, and food insecurity.
- Maintain accurate and timely documentation in the electronic medical record.
- Apply case management guidelines, regulatory requirements, and clinical best practices.
- Collaborate with interdisciplinary healthcare teams to improve patient outcomes.
- Serve as a resource and mentor to new team members when appropriate.
Minimum Qualifications
- Active, unrestricted New Jersey RN License
- BSN required (or Licensed Social Worker (LSW) if applicable)
- Minimum 3 years of recent clinical experience in one of the following:
- Acute Care Hospital
- Home Health
- Ambulatory Care
- Skilled Nursing Facility
- Strong assessment and care coordination skills
- Excellent communication and documentation abilities
- Comfortable working independently in a field-based environment
- Proficient with electronic medical records and Microsoft Office
