Description
Key Responsibilities
- Evaluate patients and develop individualized physical therapy plans of care.
- Perform comprehensive physical therapy examinations and establish diagnoses.
- Modify treatment plans based on patient response and evidence-based practice.
- Implement functional therapeutic interventions.
- Educate patients, families, caregivers, students, and healthcare staff.
- Maintain accurate, timely documentation in accordance with organizational and regulatory standards.
- Use standardized outcome measures to assess patient progress.
- Screen patients to determine the need for therapy services.
- Supervise Physical Therapist Assistants (PTAs) and rehabilitation technicians.
- Monitor delegated care and provide ongoing feedback to support personnel.
- Communicate patient progress, discharge recommendations, equipment needs, and transfer/activity recommendations to the interdisciplinary care team.
- Demonstrate safe patient handling techniques and proper body mechanics.
- Treat patients with dignity, empathy, and professionalism.
Required Qualifications
- Bachelor's Degree in Physical Therapy.
- Active Illinois Physical Therapist (PT) License.
- Basic Life Support (BLS) certification from the American Heart Association.
