Key Responsibilities

• Evaluate patients and develop individualized physical therapy plans of care.
• Perform comprehensive physical therapy examinations and establish diagnoses.
• Modify treatment plans based on patient response and evidence-based practice.
• Implement functional therapeutic interventions.
• Educate patients, families, caregivers, students, and healthcare staff.
• Maintain accurate, timely documentation in accordance with organizational and regulatory standards.
• Use standardized outcome measures to assess patient progress.
• Screen patients to determine the need for therapy services.
• Supervise Physical Therapist Assistants (PTAs) and rehabilitation technicians.
• Monitor delegated care and provide ongoing feedback to support personnel.
• Communicate patient progress, discharge recommendations, equipment needs, and transfer/activity recommendations to the interdisciplinary care team.
• Demonstrate safe patient handling techniques and proper body mechanics.
• Treat patients with dignity, empathy, and professionalism.

Required Qualifications

• Bachelor's Degree in Physical Therapy.
• Active Illinois Physical Therapist (PT) License.
• Basic Life Support (BLS) certification from the American Heart Association.
