Position Summary

Lewis County General Hospital is seeking an experienced Licensed Practical Nurse (LPN) to join its Long-Term Care (LTC) team on a 13-week travel contract. The ideal candidate will provide compassionate, high-quality nursing care to residents while working collaboratively with the interdisciplinary healthcare team.
 

Responsibilities

• Provide direct nursing care to long-term care residents
• Administer medications and treatments as prescribed
• Monitor, assess, and document patient conditions accurately
• Assist residents with daily living activities and promote their comfort and well-being
• Collaborate with physicians, registered nurses, and other healthcare professionals to develop and implement care plans
• Maintain accurate and timely patient documentation
• Follow infection control, safety, and facility policies and procedures
• Respond promptly to changes in patient condition and communicate appropriately with the healthcare team
• Deliver compassionate, patient-centered care while maintaining professional standards

Qualifications

• Active New York Licensed Practical Nurse (LPN) license
• Previous Long-Term Care (LTC) experience preferred
• Current BLS Certification
• Strong clinical assessment, communication, and organizational skills
• Ability to work effectively in a fast-paced healthcare environment
