Pride health is actively hiring a Registered Nurse to join our client's home health program in a growing care team serving patients.

Compensation:

• Local Pay: $53–$58/hour (W-2)
• Travel Pay: $2,500–$2,600 gross per week
• Mileage Reimbursement: $0.50/mile (from first patient to last patient)

Location: Forty Fort, PA, 18704

Coverage Areas – Luzerne and Lackawanna counties.

Shift schedule (days, start & end times): M-F 8-430 every other weekend on call, rotating evening on call

Type of certification/License required: Pennsylvania RN license• Productivity Expectations – 26 productivity points per week

• Guaranteed Hours – 32 hours per week

• On Call Requirement - every other weekend on call – 3 nights per week

• First Day Report Time – 9am.

• Scrub Color – Navy Blue.

Responsibilities:

• Manage hospice admissions, recertifications, discharges, and comprehensive patient assessments while developing and updating individualized care plans to ensure high-quality end-of-life care.
• Deliver skilled hospice nursing care by monitoring patient conditions, managing medications, coordinating DME and medical supplies, conducting death visits, and educating patients and caregivers.
• Maintain accurate and timely documentation in HCHB/PointClickCare (PCC), collaborate with the interdisciplinary team, and meet the productivity expectation of 26 points per week while ensuring regulatory compliance.
• Provide compassionate, patient-centered care across Luzerne and Lackawanna Counties, actively participating in the rotating on-call schedule (every other weekend and evening on-call) to ensure continuity of care.
