Required Experience
- Minimum 2 years of experience in Surgical Services specialty
- First-time travelers not accepted (prior travel experience required)
Schedule & Shifts
- Day shift: 6:30 AM – 5:00 PM (4 shifts of 10 hours)
- Guaranteed hours options: 8- or 10-hour shifts totaling 36-48 hours per week
- Weekend requirement: 1 full weekend within a 6-week call schedule
- On-call requirement: Approximately 110 calls
- Must work 2 out of 3 major holidays
- Block schedule accommodations to be discussed
Certifications & Licenses
- NBSTA (National Board of Surgical Technology and Surgical Assisting) certification required and must be current
- Indiana state license required (pending licenses not accepted)
- BLS (Basic Life Support) certification required (AHA or RQI accepted)
- Additional certifications may be required depending on cost center (e.g., ACLS, PALS, NRP, CHEMO, TNCC, EFM, NIH Stroke Certification) — verify as applicable
Required Documents & Verifications
- Valid Driver’s License or Passport (clear copy)
- Background checks including:
- 7-year residential/work history
- SSN Trace, County Criminal, Statewide, National Sex Offender, FACIS Level III
- Education verification (highest applicable education)
- EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
- Transcript or diploma (official copy required; may pend at start if needed)
- State license verification within 30 days of start
- Skills checklist completed within 12 months and annually
- Employment application covering last 7 years with explanations for gaps over 30 days
- Minimum 2 professional references from recent employment verified within 2 years (must be from charge, manager, supervisor, or above)
Health & Safety Requirements
- Physical exam completed within 12 months of start
- Fit mask test within 12 months (specific respirators approved; on-site fit testing limited)
- Negative 10-panel drug screen within 12 months (including amphetamines, marijuana, cocaine, opiates, PCP)
- Immunizations/vaccinations required:
- Rubeola (Measles): Positive titer or 2 vaccines (no declinations accepted)
- Mumps: Positive titer or 2 vaccines (no declinations accepted)
- Rubella: Positive titer or 2 vaccines (no declinations accepted)
- Varicella: Positive titer or 2 vaccines (no declinations accepted; history not accepted)
- Hepatitis B: Series, titer, or signed declination required
- Tetanus/Diphtheria/Tdap: Strongly recommended (preferred if administered at age 11 or older)
- Seasonal Flu vaccination or approved medical/religious exemption required during flu season (9/30–3/31)
- Tuberculosis screening within 1 year (PPD, QuantiFERON-TB Gold, or T-SPOT) with chest x-ray if positive
- Annual TB questionnaire required
- Color blind exam (Ishihara or equivalent) with 100% minimum score

- Charting system: Cerner
- Scrub color provided

Additional Notes
- Locals accepted
- Perioperative/OR travelers exempt from certain state RN restrictions effective 11/30/24
- Must complete facility-specific orientation, compliance, confidentiality, and policy documents prior to start
- Mandatory safety and competency exams required
- Must complete any required facility-specific training modules by deadlines to attend orientation
Requirements/Qualifications
- Minimum 2 years of experience as a CVOR Technologist
- CVOR Technologist experience within the last year
- Completion of at least one travel assignment
- Current BLS (Basic Life Support) certification issued by the American Heart Association
