Travel Registered Nurse (RN) – Home Health | Gwinnett, GA
Pride Health is hiring a Travel Home Health Registered Nurse (RN) to support our client's healthcare organization in Gwinnett, GA. This is an excellent opportunity to join a leading home health team on a travel assignment.
Job Details
- Job Title: Travel Registered Nurse (RN) – Home Health
- Location: Atlanta, GA
Coverage area - Gwinnett - Schedule: Monday–Friday | 8:00 AM – 4:30 PM
- Weekly Hours: 32
- Productivity: 30 points/week
- On-Call: Once per month
- Mileage Reimbursement: Yes (between patient visits)
Responsibilities
- Provide skilled nursing visits in patients' homes.
- Complete comprehensive patient assessments and OASIS documentation.
- Develop and update individualized care plans.
- Perform medication management, wound care, and patient education.
- Document visits accurately using Homecare Homebase (HCHB).
- Collaborate with physicians and the interdisciplinary care team.
Qualifications
- Active Georgia RN License.
- Home Health experience required.
- OASIS and Homecare Homebase (HCHB) experience required.
- Strong assessment, communication, and clinical skills.
- Valid driver's license and reliable transportation.
