Medical Social Worker (MSW)

We are seeking a compassionate Medical Social Worker (MSW) with experience in acute care hospital case management. The ideal candidate will be responsible for psychosocial assessments, care coordination, discharge planning, patient advocacy, and connecting patients with community resources. Experience working with multidisciplinary teams, documentation, and case management in an inpatient setting is preferred. Candidates with psychiatric, emergency department, ICU, or medical-surgical experience are encouraged to apply.
