Description
Responsibilities
- Complete Start of Care (SOC), Recertification, Resumption of Care (ROC), Discharge, and other OASIS assessments.
- Perform routine skilled nursing visits for home health patients.
- Assess patient conditions and develop individualized plans of care.
- Administer medications, wound care, and other skilled nursing interventions.
- Educate patients and caregivers regarding disease management, medications, and home care.
- Coordinate care with physicians, therapists, and the interdisciplinary team.
- Complete accurate documentation in Homecare Homebase software
- Maintain productivity expectations while providing exceptional patient care.
Coverage Area
- Vermilion Parish, Louisiana
Schedule & Productivity
- Monday–Friday
- 40 hours per week
- 30 productivity points per week
- On-call: 1–2 days per week
- Weekend rotation: 1 weekend per month
