1.

Shift Detail (if variable, please list what primary shifts will be covered), list specific times: 1800-0630 evening shift into night shift.

2.

Are shifts 8s or 12s? 12s with one 8hr shift.

3.

Weekend Coverage/Rotation (i.e.

Every other, every 3rd): Every other weekend.

4.

Holiday Coverage/Rotation: every other Holiday.

5.

Length of Assignment : 13 weeks.

6.

Weekly guaranteed hours: 44hr one week 36 the 2nd week, 80hrs per pay period.

7.

Is Overtime available for this location? yes.

8.

Targeted State Date: May 10th.

9.

For RN needs, can LPNs be utilize.

- Local rates apply within 80 miles or if using GSS housing (-$8.00 to regular and OT bill rates).

- Backouts & quits with less than 14-days notice incur a $500 fee.

- IV-C Required for LPNs?: No, no IV care.

- Weekend requirement:   Every other.

- Holidays:  Every other.

- Shifts (8s or 12s): primarily 8's but willing to do 12s.
