LVN/LPN – Long-Term Care (LTC) / Skilled Nursing Facility

Position Overview

We are seeking an experienced Licensed Vocational Nurse (LVN/LPN) to provide compassionate, high-quality care in a Long-Term Care (LTC) / Skilled Nursing Facility (SNF) setting. The ideal candidate will be skilled in resident assessments, medication administration, wound care, respiratory support, IV therapy, and chronic disease management while working collaboratively with the interdisciplinary care team.

Required Skills

Resident Care

• Bathing assistance (sitz, tub, bed, and shower)
• Mouth care
• Pressure ulcer assessment and prevention
• Simple dressing changes
• Wound care and wound VAC management
• JP and Hemovac drain care

Respiratory Care

• BiPAP/CPAP management
• Oxygen administration
• Nasal cannula and face mask management
• Oral suctioning
• Tracheostomy care and management
• Ventilator care

Endocrine/Metabolic

• Blood glucose monitoring and testing

Gastrointestinal Care

• Colostomy and ileostomy care
• Feeding tube care and management

Genitourinary/Renal

• Male and female Foley catheter insertion
• Clean-catch urine specimen collection and patient instruction
• Pre- and post-hemodialysis patient management

IV Therapy & Phlebotomy

• IV pump operation
• Phlebotomy
• Starting and maintaining peripheral IVs

Cardiac Monitoring

• Obtain and perform 12-lead EKG/ECG

Qualifications

• Active LVN/LPN license
• Experience in Long-Term Care, Skilled Nursing, or Nursing Home settings preferred
• Strong clinical assessment, communication, and documentation skills
• Ability to work collaboratively in a fast-paced resident care environment
  Charting: EPIC experience required
