Travel Occupational Therapist (OT) – Long-Term Care
Location: Moore, Oklahoma
Duration: 13 Weeks
Setting: Long-Term Care (LTC) / Rehabilitation
Schedule: Monday–Friday, 8-hour day shifts (35 guaranteed hours per week). Flexible start times between 6:00 AM and 10:00 AM, with a consistent schedule coordinated with the Director of Rehabilitation.
Weekend Requirement: None
On-Call: As needed
Position Overview
We are seeking a compassionate and skilled Occupational Therapist (OT) for a travel assignment in a long-term care and rehabilitation setting. In this role, you will evaluate, develop, and implement individualized treatment plans for patients experiencing cognitive, physical, and activities of daily living (ADL) limitations. The ideal candidate will be passionate about helping patients maximize independence, improve functional abilities, and enhance overall quality of life.
Key Responsibilities
- Evaluate and assess patients with cognitive and/or physical functioning impairments and develop individualized plans of care.
- Utilize standardized assessments and evidence-based practices to guide treatment interventions.
- Provide skilled occupational therapy services in accordance with regulatory requirements and clinical best practices.
- Implement therapeutic interventions including:
- ADL retraining
- Therapeutic activities
- Therapeutic exercises
- Modalities
- Training in supportive and assistive devices
- Monitor patient progress and modify treatment plans as appropriate.
- Evaluate treatment effectiveness and make recommendations based on outcomes.
- Educate and counsel patients, families, caregivers, and healthcare team members regarding treatment plans and functional goals.
- Collaborate with interdisciplinary team members to ensure coordinated patient care.
- Complete discharge planning and comprehensive discharge summaries.
- Maintain accurate and timely documentation within the electronic medical record.
- Record treatment minutes and services provided in compliance with clinical and regulatory standards.
- Participate in patient care conferences, rehabilitation meetings, utilization reviews, and other required team meetings.
- Attend required trainings, in-services, and continuing education activities.
- Assist in obtaining necessary adaptive equipment, durable medical equipment (DME), and therapy supplies.
- Supervise and support Occupational Therapy Assistants (OTAs), rehabilitation aides, and students as appropriate.
- Adhere to NBCOT standards, state practice acts, professional ethics, and organizational policies.
Qualifications
- Active Oklahoma Occupational Therapist (OT) license.
- Minimum of 1 year of Occupational Therapy experience required.
- Previous Long-Term Care (LTC) and/or rehabilitation experience required.
- Strong clinical assessment, documentation, communication, and patient care skills.
- New graduates may be considered if they have completed a Level II Fieldwork experience.
