Ā Contract Length: 13 Weeks
Ā Schedule: Day Shift | 5Ć8 Hours (8:00 AM ā 5:00 PM)
Ā Hours: 40 Hours/Week

Position Summary
Seeking an experienced Physical Therapist (PT) to provide skilled home health services to adult and geriatric patients. The PT will assess patients, develop treatment plans, improve functional mobility, and help patients achieve maximum independence in their home environment.Ā
Ā
Key Responsibilities
Ā Perform PT evaluations and reassessments
Ā Develop individualized treatment plans and goals
Ā Provide gait, balance, strength, and mobility trainingĀ
Ā Complete OASIS and home health documentation accuratelyĀ
Ā Assess home safety and identify environmental barriersĀ
Ā Monitor pain levels, vital signs, and patient progressĀ
Ā Educate patients and caregivers on exercises and safety measuresĀ
Ā Collaborate with healthcare teams to ensure quality patient care š¤
Ā Manage approximately 6 patient visits per day

Required Qualifications
āļø Active Physical Therapist License
āļø Minimum 2 years of PT experience
āļø BLS Certification š©ŗ
āļø Strong documentation and time-management skills ā°
āļø Home Health experience preferredĀ

Ā W2 Hourly Rate: $26/hr
Ā OT Rate: $39/hr
