Description
Schedule
- Shift: Monday–Friday
- Hours: 8:00 AM – 4:30 PM
- Hours: 40 hours/week
- Productivity Requirement: 30 points/week
- On Call/Weekend Requirement: One weekend per month if needed
- Coverage Area: Sumner, Davidson, and Wilson Counties
- Mileage Reimbursement: Yes – mileage reimbursed between patient visits
Required Qualifications
- Active RN license in the State of Tennessee.
- Minimum 3 years of recent Home Health RN experience.
- Recent Homecare Homebase (HCHB) experience required.
- Competent with OASIS assessments.
- Strong clinical assessment and documentation skills.
